Adrian Monk (Tony Shalhoub) es un ex-policía de San Francisco que se hizo famoso por los métodos poco convencionales con los que resolvía los casos más complicados. Pero el asesinato de su esposa le convirtió en un maníaco obsesionado por todo lo que le rodea (gérmenes, alturas, multitudes, espacios cerrados, alimentos...), lo que hizo que fuera expulsado del cuerpo. Ahora Adrian es un detective privado que necesita ayuda hasta para ordenar el cajón de los calcetines, sencilla tarea que su complicada mente puede convertir en un reto angustioso. Afortunadamente para él, cuenta con la incondicional ayuda de Sharona (Bitty Schram), su enfermera, que es quien le ayuda tanto para poder enfrentarse a la vida diaria como en los casos que tienen en jaque a la policía. Su ex-jefe, por mucho que le pese, acude a Monk en muchas ocasiones para que aporte una visión alternativa, ya que sus neurosis le ayudan a desentramar crímenes aparentemente irresolubles desde una perspectiva distinta.
